{"id":6889,"date":"2020-11-04T11:19:00","date_gmt":"2020-11-04T07:19:00","guid":{"rendered":"https:\/\/www.medisysdata.com\/?p=6889"},"modified":"2022-11-01T14:11:05","modified_gmt":"2022-11-01T08:41:05","slug":"when-is-it-appropriate-to-use-modifier-90","status":"publish","type":"post","link":"https:\/\/www.medisysdata.com\/blog\/when-is-it-appropriate-to-use-modifier-90\/","title":{"rendered":"When is it appropriate to use modifier 90?"},"content":{"rendered":"\r\n<h2 class=\"wp-block-heading\"><strong>Reference (Outside) Laboratory<\/strong><\/h2>\r\n\r\n\r\n\r\n<p>When laboratory procedures are performed by a party other than the treating or reporting physician, the procedure may be identified by adding the modifier 90 to the usual procedure number.<\/p>\r\n\r\n\r\n\r\n<p>Modifier 90 should be <a href=\"https:\/\/med.noridianmedicare.com\/web\/jeb\/topics\/modifiers\/90\">appended <\/a>to the procedure code representing the laboratory service that is being submitted to an outside laboratory for processing in lieu of being processed within the practice or clinic of the treating\/reporting physician or other qualified health care professional. Typically, the clinician reports the laboratory service but the actual testing occurs at an outside laboratory.<\/p>\r\n\r\n\r\n\r\n<h2 class=\"wp-block-heading\"><strong>Correct Use<\/strong><\/h2>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Outside laboratory performs procedure, unrelated to treating\/reporting physician\r\n<ul>\r\n<li>In most cases, lab furnishing the service would bill the claim<\/li>\r\n<\/ul>\r\n<\/li>\r\n<li>Possible for one lab to bill service performed by another lab\r\n<ul>\r\n<li>Referring = referring specimen to another laboratory for testing<\/li>\r\n<\/ul>\r\n<ul>\r\n<li>Reference = lab that receives specimen from another lab and performs one or more tests on such specimen<\/li>\r\n<\/ul>\r\n<\/li>\r\n<li>Must append modifier 90 to referred laboratory test code\r\n<ul>\r\n<li>Item 20 mark &#8220;Yes&#8221; = outside lab<\/li>\r\n<\/ul>\r\n<ul>\r\n<li>Purchase price must be reflected under charges<\/li>\r\n<\/ul>\r\n<ul>\r\n<li>Complete item 32 with NPI, name and address where performed<\/li>\r\n<\/ul>\r\n<\/li>\r\n<li>Appropriate modifier 90 claims include two different Clinical Lab Improvement Amendment (CLIA) numbers\r\n<ul>\r\n<li>Reflect billing provider information<\/li>\r\n<\/ul>\r\n<ul>\r\n<li>Laboratory where services were performed (reference lab)<\/li>\r\n<\/ul>\r\n<\/li>\r\n<li>\u00a0\u00a0\u00a0\u00a0Bill claims with modifier 90 and without modifier 90 separately<\/li>\r\n<li>\u00a0\u00a0\u00a0\u00a0If no purchased services, leave item 20 blank<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<h2 class=\"wp-block-heading\"><strong>Do not Use Modifier 90 in below cases:<\/strong><\/h2>\r\n\r\n\r\n\r\n<ul class=\"wp-block-list\">\r\n<li>Do not report modifier 90 with anatomic pathology and lab services<\/li>\r\n<li>Do not append modifier 90 for drawing fee (36415)\r\n<ul>\r\n<li>Cannot be referenced out to another lab<\/li>\r\n<\/ul>\r\n<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p>Modifier 90 is used by a physician or clinic when the laboratory tests performed for a patient are performed by an outside or reference laboratory.
